CHRISTUS Spohn Health Network

How to Become a Provider

By becoming a participating provider with the CHRISTUS Spohn Health Network (CSHN), you become part of a quality network of providers with local provider relations representatives and a medical director who oversee credentialing and medical management.

Participating providers identify the network's members by the CSHN logo imprinted on the members' identification cards. CSHN members include many large, local employers, such as school districts, public and private businesses, and CHRISTUS Spohn Health System employees.

Interested in becoming a participating provider?

Physicians interested in becoming participating providers with the CHRISTUS Spohn Health Network should:

  • Have unrestricted, active, provisional or affiliated network staff membership at one of the CHRISTUS Spohn Health Network participating hospitals.
  • Hold a current, unrestricted Texas medical license as defined by the Texas State Board of Medical Examiners.
  • Practice within the CHRISTUS Spohn Health Network geographical service area. (See Maps/Facility Locator).

All nonphysician practitioners must:

  • Hold a current, unrestricted Texas license as defined by their specialty and/or by the applicable state licensing authority.
  • Have a master's or higher degree and have graduated from an accredited university or college, if required.
  • Practice within the CHRISTUS Spohn Health Network geographical service area (see Maps/Facility Locator).
  • Meet any other specific criteria that need to be satisfied for a specialty.
